Over the next few weeks I’m going to share details and paperwork, contracts, procedures, etc. that we use in our office. Today I’ve uploaded all the paperwork we use when starting a case: the contract, informed consent, photo release, appointment policy, no-no list and the financing paperwork including the lifetime retainer option. There are lots of ways to do things and I’m certainly not saying we are right or that this is the only way but I hope you find this useful.   3. no exclusions. two sets of retainers a year.

  4. for how long? for a life time? what about fixed retainers? do you include them as well?

  5. lifetime retainers are for essix type retainers only. Bonded retainers are not covered. Yes for as long as the patient is around and the practice is around we will replace their retainers. You would be surprised by how rarely there is call for replacement.

  6. what about those situations when a patient paid for life long retention and needs alignment due to relapse? any special arrangements there? thanks
